We read with interest the Seminar by Mervyn Singer and colleagues,1 which highlights advances and ongoing challenges in sepsis pathobiology and therapeutics. The authors discuss immunosuppression as a key modifier of sepsis risk, clinical presentation, and outcomes, particularly among individuals with organ transplantation, autoimmune disease, and malignancy.1 However, despite being a major determinant of sepsis epidemiology and outcomes in high-burden settings, HIV co-infection is left unmentioned
